ALTA SURVEY - TABLE A ITEMS (Checked Items are Normally Asked For, Remove if NOT Needed.)
1. MONUMENTS PLACED at all major corners of the boundary of the property, unless already marked.
2. ADDRESS(ES) of the surveyed property if disclosed or observed while conducting the survey.
3. FLOOD ZONE CLASSIFICATION depicted by scaled map location and graphic plotting only.
4. GROSS LAND AREA.
5. VERTICAL RELIEF with the source of information, contour interval, datum, and originating benchmark when appropriate.
6.(a) LIST the current zoning classification, as noted in the Zoning Report provided to the surveyor.
6.(b) GRAPHICALLY DEPICT the building setback requirements, as noted in the Zoning Report provided to the surveyor.
7.(a) EXTERIOR DIMENSIONS of all buildings at ground level.
7.(b)(1) SQUARE FOOTAGE of exterior footprint of all buildings at ground level.
7.(c) MEASURED HEIGHT of all buildings above grade. Identify location of measurement.
8. SUBSTANTIAL FEATURES observed while conducting the survey such as parking lots, billboards, signs, swimming pools, landscaped areas, etc.
9. PARKING: Striping, number and type of parking spaces.
10. WALLS: Determination of the relationship and location of certain division or party walls designated by the client with respect to adjoining properties.
11a. UTILITIES PROVIDED: Location of utilities existing on or serving the surveyed property as determined by PLANS and/or REPORTS.

: Location of utilities existing on or serving the surveyed property as determined by PRIVATE UTILITY LOCATE REQUEST.
12. GOVERNMENTAL AGENCY survey-related requirements as specified by the client (HUD, Bureau of Land Management leases, etc.)

14. DISTANCE to the NEAREST INTERSECTING STREET as specified by the client.
15. ORTHOPHOTOGRAPHY, photogrammetric mapping, airborne/mobile laser scanning OR drone photo process to locate certain features (excluding boundaries.)
16. EVIDENCE of recent earth moving work, building construction or building additions.
17. PROPOSED CHANGES in STREET RIGHT OF WAY lines, if information is made available from the controlling jurisdiction. Evidence of recent street or sidewalk construction or repairs.
18. OFFSITE EASEMENTS: Include any plottable offsite easements disclosed in documents provided to or obtained by the surveyor.
19. PROFESSIONAL LIABILITY INSURANCE policy, minimum amount of $____________, in effect through contract term. Cert. of Insurance furnished upon request.
